Your medication list

Bring the names and doses of everything you currently take.

That includes prescription medications, over-the-counter medications, vitamins, and supplements.

An easy option: take pictures of your prescription bottles with your phone.

Information about medications you've tried before

If you've previously taken semaglutide, tirzepatide, phentermine, or another weight-loss medication, it helps to know:

  • What you took

  • Your previous dose

  • When you stopped

  • Why you stopped

  • Whether you experienced side effects

Don't guess if you don't remember. Tell us what you know.

Your identification

Having your current ID available can prevent unnecessary delays with paperwork or prescriptions.

Questions you've been meaning to ask

Write them in your Notes app before you arrive.

Common questions include:
