  • All you need is a valid Oklahoma ID or Driver’s License! If your address isn’t current, no worries—we can use a separate mailing address. And for our fabulous friends with Medicare, Soonercare, or if you're a 100% disabled veteran, please bring along proof of that (like a card, a screenshot, or a letter from the VA). We're all about making this as smooth as possible!

  • We take walk-ins every day we are open.

    Tuesday 9am to 5pm, we stop seeing patients at 4.

    Friday 9am to 5pm, we stop seeing patients at 4.

    Saturday 9am to 12pm, we stop seeing patients at 11.

    We ask that you arrive at least one hour prior to closing to give us enough time to see you and process your application.

Upon arrival, you'll be greeted with a warm welcome and handed some new patient paperwork. Don’t fret; it's just to get to know you better! After that, our fabulous doctor will chat with you about your health history to see if you’re a candidate for medical marijuana. Spoiler alert: in Oklahoma, everyone’s a candidate! Yes, you heard that right—no qualifying conditions needed! Your appointment will be a brief 15-minute chat, but just a heads up, plan to stick around our cozy little spot for about 30-45 minutes if you want our awesome staff to help you through the registration process. Easy peasy, right? Get ready for a smooth experience!

Come Prepared

To apply for the two-year medical marijuana license in Oklahoma, you must be a resident of the state. Upon arrival, please be prepared to provide the following information:

Driver’s license or state-issued I.D.

If your driver’s license does not reflect your current physical address, please inform our staff.

Patient’s full legal name

Residence and mailing address

Date of birth

Telephone number and email address

Brief health history

We’re here to help you! Here’s what we’ll need from you:

Proof of Residency: Grab any government-issued photo ID. No need to wrestle with utility bills or lease agreements—it’s super easy!

Proof of Identity: Just show your shining self with any form of ID that shouts, “Yes, that’s me!”

Medicaid/Medicare, Sooner Care: If it applies to you, don’t forget to bring your enrollment documentation. Let’s keep it classy and covered!

VA Letter (100% Disability): For our veterans, a letter stating your 100% disability will do. Your service is vital, and we're here for you!

Now, after you’ve dotted those i's and crossed those t's, remember that our physician’s form needs to shimmy its way into OMMA’s online database within 30 days. Easy peasy, right?

 

FAQs

Do I need any records from my primary care physician?

You do not. Oklahoma does not require any preexisting conditions to apply for your license. Our doctor will, however, ask you what symptoms you will be treating with cannabis.

How long does it take to receive my card?

After submitting your application online, you should receive email confirmations as your application makes its way through the process. Typically patients who are approved will get their card in the mail within 14-18 business days.

How much does a patient recommendation cost?

The standard cost of the license is $104.30. This fee charged when you apply with your signed recommendation form online. Sooner care or medicaid patients and veterans who are 100% disabled pay a discounted fee of $22.50.

How do I renew my license?

You will see the doctor just as you did for your original license. Same form, same state fees. We will need a copy of your drivers license or state issued I.D. You do not need an appointment. Visits typically take less than 30 minutes.

What happens after I apply?

Once you have seen our doctor and received your signed recommendation form, we will create an account on OMMA’s website and upload all of your documentation. State fees are accepted through the OMMA’s website so a debit/credit card is required.

How long is my license good for?

Oklahoma medical marijuana licenses are good for two years after the date it was issued which is listed on the front of your medical card.
